Insufficient evidence is available to reliably compare the roles of serum alkaline phosphatase (ALP) and bone mineral density (BMD) in breast cancer patients. This study aimed to compare associations between high levels of ALP and BMD in Postmenopausal with and without metastatic breast cancer patients to elucidate the importance of avoiding metastatic breast cancer risk.


Background To evaluate total body bone mineral density and body bone mineral content in patients with breast cancer with and without metastases, and to compare them with serum alkaline phosphatase findings.

Methods 30 patients with breast cancer and 15 healthy subjects were investigated by dual- energy x-ray absorptiometry and serum alkaline phosphatase analysis by p-Nitrophenyl Phosphate method with stabilized substrate.


Results Our results show that the patients with bone metastases breast cancer had higher total bone mineral density than healthy controls and breast cancer patients without bone metastases and associated with increased risk of breast cancer, while bone mineral content was not clear to show  the difference between three groups. There was a significant difference between three study groups in serum alkaline phosphatase (p=0.001). Serum alkaline phosphatase in metastatic breast cancer patients was higher than non metastatic group and controls. The statistical differences between cases and control were determined by student independent sample t-test and one way analysis of variance (ANOVA). In order to determine correlation, statistical analysis was carried out by using spearman’s rank correlation coefficient. Alkaline phosphatase was found statistically significant (p<0.05). Alkaline phosphatase activity gradually increases as advancement of stages which showed that it can be utilized as a predictor for breast cancer metastasis.


ConclusionsWe concluded from the study that the higher Total Bone Mineral Density measuring and Serum Alkaline Phosphatase level are initial indicators for metastatic breast cancer. In the future, we can simulate breast cancer mechanism to predict metastatic breast cancer before spreading by solving differential equations and utilizes various modeling Programs.
